    Keep all receipts and records

    Ask the doctor's office for an itemized bill showing the services, date, provider name, and the billing codes (called CPT codes). A generic credit card receipt is not enough.

    Sign in to your insurance member portal

    Go to your insurance company's website — like UnitedHealthcare, Blue Cross Blue Shield, or Cigna — and sign in. Look in the menu for "Claims" or "Submit a claim."

    Fill out the claim form

    You will enter the patient name, provider name, service date, and the amount you paid. Attach a photo or scan of the itemized bill and your receipt showing payment.

    Double-check before submitting

    Verify the numbers match your bill. Missing or wrong info is the main reason claims get rejected and have to be refiled.

    Watch for your Explanation of Benefits

    Within 2 to 4 weeks, the insurer sends an Explanation of Benefits (EOB) showing how much they will pay back. If they deny it, call the number on the EOB and ask why — most denials are fixable.
